Severe throbbing pain in the lower right first molar, worse at night, with pain on biting. Radiographs showed deep decay reaching the pulp with early periapical change. The patient had been managing on painkillers for a week.
The tooth was restorable, so extraction was not the right answer. We planned a single-visit root canal under full anaesthesia using rotary instruments, followed by a ceramic crown once the tooth settled.
