Come in today if you have

  • Severe toothache that painkillers are no longer controlling
  • Facial or gum swelling — this is an infection spreading and it is urgent
  • A knocked-out permanent tooth — see below, minutes matter
  • A broken or fractured tooth, especially with a sharp edge or exposed nerve
  • Bleeding that will not stop after an extraction
  • A lost crown, filling or temporary leaving the tooth exposed
  • Trauma to the mouth after a fall or accident
  • A dental abscess — a painful swelling with pus

Go to a hospital emergency department instead if

Swelling is closing your eye, spreading down your neck, or making it difficult to swallow or breathe; you have a high fever with facial swelling; or you have had significant facial trauma or a suspected jaw fracture. Those need hospital care first. Come to us afterwards for the tooth.

Knocked-out tooth — what to do in the next 30 minutes

  • Find the tooth and pick it up by the crown, never the root.
  • If it is dirty, rinse it briefly in milk or saline. Do not scrub it and do not use soap or antiseptic.
  • If you can, put it straight back into the socket the right way round and bite gently on a clean cloth.
  • If you cannot reinsert it, keep it in milk — or in the patient's own saliva. Never in plain water, and never dry.
  • Get to us immediately. A tooth replanted within 30 minutes has a good chance of surviving. After a couple of hours, very little.

Knocked-out baby teeth are never replanted. Come in anyway so we can check the socket and the adult tooth developing underneath.

While you are waiting to be seen

  • Take your usual painkiller at the correct dose
  • Hold a cold compress against the outside of the cheek for swelling
  • Rinse gently with warm salt water
  • Keep your head elevated — lying flat makes throbbing worse
  • Avoid very hot, very cold and very sweet food
  • Do not put aspirin directly on the gum; it burns the tissue
  • Do not apply clove oil to an open cavity in large amounts
  • Do not try to lance a swelling yourself

Antibiotics are not the answer on their own

A course of antibiotics from a pharmacy may dull an abscess for a week, but it does not remove the cause. The infection comes back, usually worse, and each round makes it harder to treat. The tooth needs draining, root canal treatment or extraction. Antibiotics are sometimes a useful adjunct, never the fix.
